Alternative formats

All of our forms are available in alternative formats such as Braille, large print or audio. If a student needs these, they can request them by emailing brailleandlargefont@slc.co.uk with:

  • their name
  • address
  • details of the format they need

Or they can call 0141 243 3686 (this number is only for alternative format requests).

Getting the right application form

There are two types of application form to apply for Disabled Students’ Allowance (DSA). The form they need to complete depends on the type of course they’re studying and if they’ve applied for any other student finance. If they don’t complete the correct form it will delay their application for DSA.

DSA 1 Full form (DSA1)

Students should complete this form if they’re a part-time student applying for DSA.

Claim form

Students looking to claim costs relating to their General Allowance or Travel Allowance should complete this form and return it with their receipts.

Disability Evidence form

To be completed and returned by both the student and a medical professional where the student isn’t able to give us enough evidence of their disability.

Request for Temporary DSA Support form

To be completed and returned where a university or college can’t give support and the student has submitted a formal appeal.
